Application Checklist

Please take a moment to review the checklist below to ensure that you have included every form necessary to register for your summer class.

Application/Course Registration (All students must fill out this form. A social security number is needed to assure the accuracy of a student's academic record. The social security number will be used to generate a unique 7-digit student ID number for identification which will be used on all subsequent correspondence from the University.)
Housing Form (All students requesting housing)
Payment Form (All students must fill out this form with the exception of consortium students.)
ASL/Interpreting Questionnaire (only for those taking an ASL or Interpreting course)
Medical History Form (All students attending classes on campus must provide a completed history of immunizations the first time they apply.)
BA/BS Transcript (for those taking a course where a degree is a prerequisite. If you've provided a copy of your transcript within the past three years, you do not need to send another copy.)
Payment (check, money order, visa/mastercard, tuition waiver, purchase order, or letter of authorization)
Tuition Waiver (for eligible employees of Gallaudet University and their family members must be signed by student and supervisor.
Visa Information. For international students only if attending class on campus. Be sure to attach copies of required documentation and return with your application/registration and payment forms.
