Application/Course Registration (All students must fill out this form. A social security number is needed to assure the accuracy of a student's academic record. The social security number will be used to generate a unique 7-digit student ID number for identification which will be used on all subsequent correspondence from the University.)

Tuition Waiver (for eligible employees of Gallaudet University and their family members must be signed by student and supervisor.
